Point 22 is the privacy notice on behalf of human service department regarding privacy and security of your personal information. Under Point 23, you need to sign the form and also need to mention the date of signing the form.
Ques 12- Before submitting an application make sure that, you go through the checklist. (What to mention in Point 21)
Point 21 includes the checklist which needs to be ticked if applicable to applicant. Each point should be thoroughly considered.
Ques 11- Is the applicant leaving Australia permanently before the end of the current financial year? (What should you mention in Point 20)
Choose YES, if you will leave from Australia in the financial year mentioned in Point 19 and mention expected date of departure from Australia. Otherwise choose NO if they have not left Australia in the financial year mentioned in Point 19.
Ques 10- Which financial year is the applicant applying for? (What should you mention in the Point 19?
The financial year (1 July – 30 Jun) for which you want to apply for Medicare levy exemption need to be entered.
Ques 9- Has the applicant lodged an application for permanent residency with the Department of Home Affairs under parent category (Aged parent or Contributory parent)? (What should you mention in Point 18)
Choose YES, if you have filed the permanent residency application under the parent category to home affairs (Provide the category as aged parent or contributory parents). Otherwise choose NO.
Ques 8- Has the applicant lodged an application for permanent residency (other than a parent visa) with the Department of Home Affairs, this includes applications for Spouse Combined visas (subclass 820/801) and (subclass 309/100)? (What should you select in Point 17)
Choose NO, if you have not applied for the permanent residency, and directly shifted to Point 18. Choose YES if you have applied for permanent residency. If you choose YES then provide the following details: Application 1 Date of application ...
Ques 7- Is the applicant from Finland, Malta or Norway? (What should you select in Point 16)
Choose YES if you are from Finland, Malta, or Norway Country, choose YES again if you are on student visa else NO. Otherwise choose NO.
Ques 6- Does the applicant hold current or expired health or medical insurance in Belgium, Italy, the Netherlands, Norway or Slovenia? (What should you select in the Point 15?
Choose YES if you have any expired or current health or medical insurance from the country mentioned in Point 15. Otherwise tick on NO.
Ques 5- What country was the applicant living in for 6 months or more before arriving in Australia? And how long were you residing in that country? (What to mention in the Point 13 & 14)
In Point 13 mention the name of the country from where you have come from to Australia for the first time (where you have resided 6 months or more). Mention the period (how long) of residency in the country mentioned in Point 13.
Ques 4- Applicant’s residential address in Australia? And if the postal address provided in question 11 your tax agent’s address? (Which address should you provide in Point 10, and what to select in Point 12)

If you have received any Medicare card which is currently valid or expired, then mark YES and provide the card details. If no Medicare card has been received till now, then mark it as NO
