1. Enter your first name and last name as per passport record, if your first name and last name is mentioned together in the Last Name category or vise versa, mention your name accordingly and also attach the passport copy for us to refer to.

  Choose YES, if you will leave from Australia in the financial year mentioned in Point 19 and mention expected date of departure from Australia.  Otherwise choose NO if they have not left Australia in the financial year mentioned in Point 19.
